Rockbridge Circuit Court: Passport Information

Requirements for Passport (first time applicants)

1. Must have completed passport application form DSP-11 (available at clerk's office).
2. Must have certified copy of birth certificate. (Certified copies are issued by the state health department and have raised/embossed seals. Hospital "birth registration" certificates are not acceptable. Birth "certificates" issued by the local health department or hospital are not acceptable.)
3. Must have two (2) recent passport photographs. (Photographs must be color, and they must meet specifications as to overall size and measurements from tip of chin to top of head).
4. Must have drivers' license or photo identification.
5. Applicants must appear in person to sign application in presence of clerk's office personnel.
6. Fees vary depending on age of applicant. Must have personal check or money order payable to "U.S. Department of State." Separate processing fee of $30 is payable to clerk's office and may be paid by check or cash.
7. Parents must sign for children until they are 18 years of age.
8. DO NOT SIGN APPLICATION UNTIL PRESENTED IN CLERK'S OFFICE

Requirements for Passport Renewal

1. Must complete passport renewal application form DSP-82 (available at clerk's office).
2. Must submit most recent passport, which must have been issued:
   1. less than 15 years ago, and
   2. when applicant was at least 16 years of age, and
   3. in same name as currently used on renewal application (or you must submit certified copy of your marriage license or court order changing your name to verify the reason for different name from last passport).
3. Must have two (2) recent passport photographs that meet specifications prescribed by Passport Agency.
4. Renewal application DSP-82 is mailed directly to the address on the back of the form with a $67 check, payable to "Passport Services", most recent passport and two passport photographs.
